Delta Dental Sweepstakes
Official Rules

No purchase or payment is necessary to enter or win.  A purchase will not increase your chances of winning.  Void where prohibited by law, rule, or regulation. 

Delta Dental’s sweepstakes consists of multiple prize drawings and is offered only to U.S. residents who are 18 and over on the date of entry.  The sweepstakes begins at 12:01 a.m. EDT on August 30, 2009 and ends at 11:59 p.m. EDT on September 26, 2010.  Employees, officers, and directors of Delta Dental and its affiliates and subsidiaries, their immediate families, and those living in their households are not eligible to participate.  One entry per household. 

Current Delta Dental primary subscribers may enter the sweepstakes by visiting the link available at www.deltadentalmi.com, www.deltadentaloh.com, or www.deltadentalin.com, or by logging on directly to the Consumer Toolkit at https://www.toolkitsonline.com/ipWeb/appmanager/ct/desktop, and electing electronic delivery of Explanation of Benefits statements.  Primary subscribers also may enter the sweepstakes by simply mailing name, address, and telephone number information on a 3 x 5 inch card to: Delta Dental Sweepstakes, Delta Dental Corporate Communications, PO Box 30416, Lansing, MI  48909-7916.  Each entry must be in a separate, postage paid and postmarked envelope.  Odds of winning depend solely on number of entries received. 

The Sweepstakes consists of four (4) entry periods as follows: 

Winners will be selected in random drawings according to the prize drawing schedule above from among all eligible entries received for the applicable entry period.  Online entries must be received and mail-in entries must be postmarked by the applicable entry period deadline as specified above. 

Winners will be notified within 15 days of each drawing.  Winners must comply with all instructions in the prize notification letter, including completion of an affidavit of eligibility and liability/publicity release, and return the affidavit within 15 days of the date on the prize notification letter, and also must meet all of the eligibility requirements set forth within these rules, except where prohibited by law.  If these requirements are not met, the prize will be forfeited and an alternate winner will be selected.  Acceptance or use of a prize constitutes permission for Delta Dental and its affiliates to use a winner’s name and likeness for advertising and promotional purposes without compensation, unless otherwise prohibited by law.  Verified winners who meet all requirements will receive a Kindle™ wireless reading device, each valued at approximately $279.00, via overnight delivery service to the address provided to Delta Dental.

All entries must be personally entered by the individual who is listed as the entrant and no third parties may enter the sweepstakes for or on behalf of any person.  Any dispute regarding submission of any online entry will be deemed submitted by the e-mail account holder.

Delta Dental is not responsible for lost or misdirected mail or for any malfunction, failure to function, or additional costs associated with use of the prize.  Return of any prize or prize notification as undeliverable will result in disqualification and selection of an alternate winner.  Only four (4) prizes will be awarded, one for each drawing.

By participating in this sweepstakes, participants agree to release, discharge and hold harmless Delta Dental, its affiliates, subsidiaries, agents, officers, directors and employees from any damages which may arise out of participation in the promotion or out of the acceptance, use, misuse, or possession of a prize.  Liability for any applicable taxes, including all federal, state and local taxes and/or other fees or expenses that may apply to prizes are the sole responsibility of the winner. 

If for any reason the Internet portion of the sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Delta Dental that corrupt or affect the administration, security, fairness, integrity, or proper conduct of this contest, Delta Dental reserves the right, at its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify, or suspend the sweepstakes.  Delta Dental assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications-line failure, theft, or destruction or unauthorized access to, or alteration of, entries.  Delta Dental is not responsible for any problems or technical malfunction of any telephone network or lines, computer online systems, servers or providers, computer equipment, or software, or for failure of any entry to be received by Delta Dental on account of technical problems or traffic congestion on the Internet or at any Web site, or any combination thereof, including any injury or damage to participant’s or any other person’s computer related to or resulting from participation or downloading any materials in this sweepstakes.  CAUTION: ANY ATTEMPT BY A CONTESTANT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND, SHOULD SUCH AN ATTEMPT BE MADE, DELTA DENTAL RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH CONTESTANT TO THE FULLEST EXTENT PERMITTED BY LAW.

For a list of winners or winners selected to the date of request, send a request for the winners list, together with a self-addressed stamped envelope, to: Delta Dental, Attn:  Corporate Communications / Sweepstakes Winners, PO Box 30416, Lansing, MI  48909-7916, between December 8, 2009 and December 31, 2010.  Vermont residents, or other residents where permitted, may omit return postage.  Requests received after December 31, 2010 will not be honored. 

Delta Dental reserves the right to suspend or cancel the sweepstakes or modify these official rules as necessary for any reason or as required by applicable law.

Delta Dental collects personal information when entrants enter this promotion.  Entering the promotion constitutes permission for Delta Dental to collect, maintain and use this data in accordance with Delta Dental’s Privacy Statement, located by selecting the “Privacy Statement” link at www.deltadentalmi.com, www.deltadentaloh.com, or www.deltadentalin.com.
