What is the period this $1,000 credit will be offered or available?
The program will begin for eligible dentists on May 26, 2020, and end December 31, 2020. Unless Delta Dental of Michigan, Ohio, and Indiana decides to extend the credit period, the credit will not be applied to any accounts after December 31, 2020.


Do I have to be a participating Delta Dental dentist?

No, all dentists in Michigan, Ohio and Indiana who are currently practicing, and have an active license and Type 1 NPI are eligible to receive a $1,000 credit to Henry Schein.


What are the steps I must take to get the $1,000 credit that Delta Dental is offering through Henry Schein?

All outstanding applications for a Henry Schein credit provided by Delta Dental of Michigan, Ohio, and Indiana must be submitted by December 31, 2020. 

If you have an active account and have not received your credit, you must fill out this form by December 31, 2020, to apply for this credit. 

If you filled out the form previously and did not receive your credit, the form may have been rejected due to missing or invalid information. We must receive your application with accurate data by December 31, 2020, or your credit will not be issued.

The program ends December 31, 2020, and forms will not be accepted in 2021.

If you are part of a group practice, you have two options:

  1. Request that your credit go to your group practice’s current account by filling out the form and including your practice’s Henry Schein account number. You must check the box at the bottom of the form to designate that your credit go to your group practice account.

  2. Open your own/new Henry Schein account where the credit can be applied. Call 844-269-4204 or set up your account on the Henry Schein website. Then, include your new account number in the form and fill out the top portion "Dentist Section" only.


Do I need to have an account with Henry Schein?

Yes. If you do not have an account, there are two ways to receive the credit:

  1. Request that your credit go to your group practice’s current account by filling out the form above and including your practice’s Henry Schein account number. You must check the box at the bottom of the form to designate that your credit go to your group practice account.
  2. Open your own/new Henry Schein account where the credit can be applied. Call 844-269-4204 or set up your account on the Henry Schein website. Then, include your new account number in the form above and fill out the top portion "Dentist Section" only.

Do I have to pay this back to Delta Dental?

No, this credit is being issued to you to for use at Henry Schein in an effort to help you obtain the supplies necessary to better serve your patients.


Once I have created an account with Henry Schein and complete the Delta Dental required form, how long will it take to receive the $1,000 credit?

Delta Dental will verify eligibility of dentists with new Henry Schein accounts every two weeks, and the credit will be applied by Henry Schein within approximately 30 days.


I have multiple doctors in my practice. Will the $1,000 be available to each?

Yes, each licensed, private practice dentist with an active individual NPI is eligible for the $1,000 one-time credit.


What if the dentist works in three offices or at three locations – will they receive a $1,000 credit for each office or location?

No, each eligible dentist will receive one $1,000 credit.


What if some dentists participate with Delta Dental and some do not in the same practice?

The credit is available to all Dentists in Michigan, Ohio and Indiana who are currently practicing, and have an active license and Type 1 NPI, regardless of their participation with Delta Dental networks.


I have an account with another supply chain vendor. Does that negate the $1,000 credit from Delta Dental through Henry Schein?

No, you may have an account with another supply vendor and still receive this Henry Schein credit. If you do not have a Henry Schein account, there are two ways to receive the credit:

  1. Request that your credit go to your group practice’s current account by filling out the form above and including your practice’s Henry Schein account number. You must check the box at the bottom of the form to designate that your credit go to your group practice account.
  2. Open your own/new Henry Schein account where the credit can be applied. This does not change any status you have with another supply vendor. Call 844-269-4204 or set up your account on the Henry Schein website. Then, include your new account number in the form above and fill out the top portion "Dentist Section" only.

Can I use all or part of this $1,000 credit to pay on my current Henry Schein account?

Eligible dentists can use the credit toward existing Henry Schein account balances or toward any future purchase of dental products and/or services from Henry Schein. For example: If you receive the credit and have an existing account balance of $600, you will have a $400 credit remaining for future purchases.


Can I buy anything through Henry Schein with the $1,000 credit or does it have to be PPE related?

You are free to use your credit provided by Delta Dental in any way you deem necessary for purchases through Henry Schein.
