Your Delta Dental website allows you to:

  • Access your Member Portal; (see instructions below)
  • View your plan summary
  • Print your own member ID card
  • View your Explanation of Benefits (EOB) statements including claims transactions, reimbursements, and payments

What's Member Portal?

Computer iconMember Portal gives you easy, secure access to your benefit information 24/7. Review eligibility, annual maximum used to date, levels of coverage and claims information. You can also search for a dentist, print your member ID card and sign up for electronic Explanation of Benefits (EOB) statements.

To access Member Portal, you will need to register first. To register you will need the following:

  • Subscriber's first and last name
  • Subscriber's date of birth
  • Employee ID number or Social Security number
  • Security question

 

Find a dentist

dentist iconWith your Delta Dental PPo™ (Point-of-Service) plan you can visit any licensed dentist. You will usually save the most money by choosing a participating Delta Dental PPO dentist. Use our online dentist search to find out if your current dentist is part of the Delta Dental network.

 

Print your own member ID card

id card iconTo print an ID card, you will need to log in to the Member Portal. Once logged in, click the Print ID Card link on the left-hand menu. You will see your member ID card displayed on the screen. Use your browser's print function to print as many copies as desired.

 

Download a claim form

Claim form iconRemember, when you visit a participating Delta Dental dentist, the dentist will complete and file claim forms for you. If you choose to visit a dentist who does not participate with Delta Dental, you may need to file your own claim forms. Log in to the Member Portal to download your claim forms.
