Your Delta Dental benefits through Michigan Public School Employees' Retirement System utilize the Delta Dental PPO and Delta Dental Premier Networks. Click on the Find a dentist box below and select ‘Delta Dental PPO Plus Premier’ from the Plan Network drop down menu to find participating dentists near you.

Choosing a dentist

With your dental plan, you are welcome to go to the dentist of your choice: a Delta Dental PPO™ dentist, a Delta Dental Premier® dentist or a nonparticipating dentist. However, your cost will likely be lowest when you go to a Delta Dental PPO dentist. Finding a dentist is easy, using the link below, select 'Delta Dental PPO Plus Premier' from the network menu to find a dentist near you.
Delta Dental PPO dentists
Your dental plan is designed to offer the maximum benefit with the lowest out-of-pocket expense when you use Delta Dental PPO dentists. Delta Dental PPO dentists agree to accept Delta Dental’s PPO approved amount as payment in full for covered services. You are only responsible for your coinsurance.
Delta Dental Premier dentists
Many dentists that are not part of the Delta Dental PPO network may be in the Delta Dental Premier network. You may still save money when you use a Delta Dental Premier dentist, but you will be responsible for your coinsurance plus any additional costs for using a dentist outside the Delta Dental PPO network.
Nonparticipating dentists
Dentists that do not participate with Delta Dental do not have an agreement to accept Delta Dental’s approved amounts. Nonparticipating dentists will likely cost you the most money. When you use providers that do not participate with Delta Dental, you are responsible for paying your coinsurance plus any additional costs up to the dentist’s charge. If you choose to visit a nonparticipating dentist, you may need to file your own claims. Printable claims forms can be found in the FAQ section at the bottom of the page.

Frequently asked questions

Delta Dental does not mail EOB statements for services covered at 100 percent and for which the patient owes nothing. These statements, however, are available to view and print on our secure Member Portal. Once you have logged in to Member Portal, click on Claim Info. Enter the date of service in the Start Date field, or enter a date range using Start Date and End Date. Click on the Claim Number for the EOB you wish to view. You also can click on the Procedure Code number for a definition of the service provided. To print EOB statements, click on the printer icon in your browser toolbar or select File then Print.

Information for dependents 17 and older must be obtained via phone.

You may print an ID card using our Member Portal or you may request an ID card via phone by calling 800-345-8756.

Delta Dental has an extensive list of participating providers. Visiting a participating provider may help to reduce your out-of-pocket expenses. However, if you choose to see a dentist who is not a participating provider with Delta Dental, you still may be eligible for partial reimbursement. Contact a customer service representative for more information.

Participating dentists will submit claims for you. If you choose to visit a nonparticipating dentist, you may need to file your own claims. Printable claim forms are available for download. Either you or your dentist may complete the form and attach a copy of your bill. Completed forms may be submitted via mail to: Delta Dental, PO Box 9085, Farmington Hills, MI 48333-9085.

1. Find a dentist:

2. Find out what services are covered:

Most Delta Dental members can receive dental treatment for regular and emergency care according to the terms of their Delta Dental coverage. Note: Dentists referred by AXA Assistance are not Delta Dental participating dentists. Members who are covered under any Delta Dental program that limits their coverage when they see a nonparticipating dentist will also have limited coverage when they see a dentist referred by AXA Assistance.

3. Submit a claim:

When services are provided by dentists in the AXA Assistance referral network, members must pay for their treatment and request a detailed receipt so they can submit a claim to Delta Dental for reimbursement upon returning to the U.S. Be sure the receipt includes:

  • Dentist’s name and address
  • Description of services performed (including teeth treated)
  • The charges and whether they were billed in dollars or local currency

Once claims are submitted, Delta Dental will reimburse members according to the terms and conditions of their existing coverage.
