Your DeltaCare USA member copay schedule provides a complete list of all covered
services by procedure code and the corresponding member responsibility.
 If a procedure code is not listed on the member copay schedule, it is not a covered benefit. Refer to the member copay schedule for limitations
and exclusions. Non-covered benefits include but are not limited to:
- Implants
- General anesthesia/IV sedation
- Occlusal guard

 DeltaCare USA is a nationwide DHMO plan. This is a closed network, meaning you must visit your assigned DeltaCare USA dentist for services to be covered under this plan.
Register or log in to your DeltaCare USA account at www1.deltadentalins.com/login to search for a participating DeltaCare USA dentist and designate a primary care dentist for you and your family.
Delta Dental has several different networks. Be sure to confirm your desired dentist is participating in the DeltaCare USA network specifically, and that you have been officially assigned to that provider before receiving treatment.
All DeltaCare USA members have the option to visit specialists, including orthodontists, however, you need to be referred by your assigned primary care dentist.

Register your DeltaCare USA account online

 Once enrolled, remember to register your account online at https://deltadental.pub/DCUSA-Register. There you can easily search and change your assigned dentist, print an ID card, and review your Evidence of Coverage for a description of benefits and copayments, important exclusions, and limitations.

Changing dentists

 You and your family members have the option to switch to a different in-network DeltaCare USA dentist. Each family member may select their own primary care dentist if they wish, which is unique to DeltaCare USA's DHMO plan.

To change dentists, simply log in to your online account at www1.deltadentalins.com/login and update your primary care dentist selection. Each family member's dentist change request needs to be submitted separately. If you choose to switch dentists, confirm your change has been made before receiving treatment. Services performed before the transfer has been completed will not be covered.
Dentist changes made by the 15th of the month are effective immediately. Changes made on or after the 16th of the month will be effective the first of the following month.
