We extend the reach of preventive health care

Preventing disease is the overarching aim of our health outreach program. We design outreach interventions for our clients that help people maintain good health through appropriate personal and professional preventive care.

Our outreach is data driven. When planning outreach projects, we analyze data on disease risk, literacy, cultural norms, social determinants of health and other factors. The findings from this research gives us a better understanding of the challenges faced by different populations. That knowledge helps us design more effective outreach interventions.

Our outreach is collaborative. We actively collaborate with stakeholders who can help us  connect with the populations we need to reach. Our partners include community groups, health coalitions, school systems, federally qualified health centers, health departments and other locally engaged organizations.

Our outreach is versatile. When planning health outreach interventions, it's important to recognize that a single outreach method will not fit all populations. We perform a wide range of interventions with various populations, approaches and objectives, including:

  • Motivating new members to utilize preventive care by providing screening and health risk assessment through interactive text messaging; 
  • Improving health literacy in underserved communities through educational events at local health fairs and similar venues;
  • Identifying and overcoming barriers to preventive care through telephone outreach operated in  conjunction with care coordinator services;
  • Using text messaging to provide enrollees with access to safety-net clinics and nonprofit mobile care providers; 
  • Working with health departments to help rural families with electronic screening for social determinants of heath, finding a dental home, in-school interventions and health counseling through home visits.
