We promote high value health care delivery

We create value with knowledge. We initiate value-based care programs for our clients by analyzing the disease risk, determinants of health and utilization patterns of their populations. We then apply that knowledge by proactively working with patients and providers to incentivize the best practices to prevent disease, improve health outcomes and control health care costs.

We create value through collaboration. Partnering with providers to develop value-driven care models enables clinicians to focus on cost-saving preventive practices. The goals of this collaboration include implementing patient-centered care, prioritizing quality over quantity of care and adoption of evidence-based practice standards.

We create value with integrated systems. Our value-based care projects are supported by the integrated systems of our Population Health Initiative. This set of interdependent services includes data analytics, care coordination, provider education and support, utilization management and multi-channel communication platforms. The flexibility of these services enables us to appropriately scale value-based projects to meet the needs of any client.

We create value by incentivizing prevention. One of our objectives has been to expand the utilization of preventive care by incentivizing provider and patient behavior. In a recent project, we tackled the problem of very young children missing out on early preventive dental care. Collaborating with a local health department, federally qualified health centers, pediatricians and other stakeholders, we used text messaging and direct phone contact to educate parents about the importance of early dental visits. Area dentists were provided with a monetary incentive and education to help them overcome hesitancy to treat very young children.
