We coordinate care for our client populations across a wide range of individuals, providers and organizations. Our mission is to help people overcome any personal or systemic barriers preventing them from receiving the health care they need.
How we help. Our care coordination services are performed by an experienced team of dental professionals, community health workers and outreach specialists. On a day-to-day basis, we provide health screening, disease risk assessment, help finding a dental home, appointment assistance and help with non-emergency medical transportation.
We go beyond the routine. Our care coordination team is proficient in adapting to challenging patient-specific scenarios. We are ready to provide customized personal interactions to help people deal with complicated health problems requiring complex inter-professional care.
When a child needs care: Hannah’s story
Hannah is a young autistic girl living in the Detroit metropolitan area. At the height of the COVID-19 pandemic, when dentists were facing orders to defer patient treatment, Hannah broke a tooth. Because of her autism, she needed care from specially trained dental professionals. Unfortunately, her father was unable to find a dentist who could provide this care.
As Hannah’s condition worsened, her father called Delta Dental for help. Our care coordinators promptly evaluated Hannah’s needs. Through our community contacts, they found a dental facility where Hannah promptly received the specialized care she needed.
