NPI and CHAMPS

National Provider Identifier (NPI)

Federal law (HIPAA) requires all healthcare providers and organizations to use a standard identifier—called the National Provider Identifier (NPI)—on electronic transmissions involving patient health information. 

Additionally, providers and businesses who accept Healthy Kids Dental or Healthy Michigan Plan patients must enroll their NPI number(s) in the Michigan Community Health Automated Medicaid Processing System (CHAMPS).

All electronic claims submitted to Delta Dental must include:

  • The treating dentist’s Type 1 NPI
  • The treating dentist’s state license number
  • The Tax Identification Number (TIN) of the billing dentist or practice
  • Optionally, the Type 2 NPI to identify the billing provider (e.g., group practice)

Applying for an NPI

You can apply in one of three ways:

  1. Online.
  2. By mail using Form CMS-10114. Call 800-465-3203 or TTY 800-692-2326 to request a copy.
  3. Through a vendor or clearinghouse using electronic file interchange.

If your practice bills under a business TIN, you’ll need a Type 2 NPI in addition to your Type 1 NPI.

Enrollment guides

Step 1: Get your Type 1 NPI

You can obtain a Type 1 NPI (for individual providers) through one of the following:

  • Online: Apply online using your Identity and Access Management System credentials.
  • Mail: Submit Form CMS-10114 to the NPI Enumerator. Request a copy at 800-465-3203 or email customerservice@npienumerator.com.
  • Bulk Submission: Authorize an electronic file interchange organization to apply on your behalf.

Step 2: Enroll in CHAMPS

Register your Type 1 NPI with CHAMPS for Michigan Medicaid programs (Healthy Kids Dental, Healthy Michigan Plan):

  • Visit MiLogin for Business
  • Applicant Type: Select “Rendering/Servicing Only”
  • Provider Type: Choose “Dental” and your specialty (e.g., General Dentist)
  • Delta Dental ID: Use 4181610
  • License: Enter your full 10-digit Michigan DDS license number (starts with 2901)
  • Taxonomy Codes:
    • General Dentist: 1223G0001X
    • Endodontics: 1223E0200X
    • Pediatrics: 1223P0221X
    • Prosthodontics: 1223P0700X
    • Orthodontics: 1223X0400X
    • Oral Surgery: 1223S0112X

Optional fields may be skipped unless otherwise noted. You may request retroactive enrollment (up to one year) in the comments section.

Step 3: Submit claims correctly

When billing Michigan Medicaid through Delta Dental:

  • Box 54: Type 1 NPI (individual provider)
  • Box 49: Type 2 NPI (billing entity, if applicable)

Ensure your practice management system reflects these NPIs for electronic claims.

Step 1: Register for a Type 2 NPI

A Type 2 NPI is required for incorporated entities, group practices and clinics that bill under a business name or Tax ID (TIN).

  • Apply online.
  • Need help? Call 800-465-3203 or email customerservice@npienumerator.com.
  • Login tips: Use your Identity and Access Management System credentials. If you don’t know your login, use the “Forgot User ID or Password” option. NPPES staff cannot share login details due to privacy protections.

If you see a message saying sole proprietors don’t need a Type 2 NPI, ignore it if you have a business name or TIN—you do need one.

Step 2: Enroll in SIGMA Vendor Self-Service

To complete CHAMPS enrollment, you must first obtain a Vendor Customer ID via SIGMA:

Wait three days after SIGMA enrollment before proceeding to CHAMPS.

Step 3: Register Type 2 NPI in CHAMPS

Once you have your Vendor ID, register your Type 2 NPI in CHAMPS as a group practice:

  • Visit MiLogin
  • Enrollment type: Select “New Group Practice”
  • Retroactive enrollment: You may request a retro date up to one year in the comments section
